Overview
Whilst on a placement, you can record care logs and report incidents via the Elder Hub, keeping families in the loop. We encourage you to record any incident, no matter how major or minor. If you have any questions, please feel free to get in touch for support.
Please note that care logs and reported incidents will be immediately visible to both Elder and the client’s family. It’s important to maintain professionalism when using the tool.
In the event of an emergency, please follow the appropriate protocols and prioritise the health of your client/yourself before recording an incident. You can find more information on dealing with an emergency here.
If you are confirmed for an upcoming placement, where the outgoing carer is facilitated through the Elder Platform, you'll also be able to view their entries. You can use these to familiarise yourself with the client's current circumstances.
Finding the tool
Head to the home page of the ElderHub. Underneath the placement card, you’ll see a button labelled 'Add care log' - this will take you to the care logs and incident reporting tool.
You’ll be able to edit three sections:

You can use care logs to provide the family with a summary of how your client is progressing. Please provide an answer to each question. Depending on the information provided, you may be prompted for further detail. Once you've finished, please click 'Save and close'.
Notes
Notes can be used to record observations throughout the day. Please do not use the Notes feature to report incidents. Once you've finished, please click 'Save and close'.
Incidents
Incidents are events occurring on your placement that you feel the customer should know about. We encourage carers to report incidents as soon as possible. Incidents reported through the CareLog are visible to Elder. Where safeguarding issues are flagged they are reviewed by Elder’s Designated Safeguarding Lead.
In the event of an emergency, please do not report via this tool. Instead, you should contact emergency services, then the client's family, and then when safe to do so Elder. You can find more information on dealing with an emergency here.
To report an incident, please click the red label titled 'Incident'. Select the relevant category, and use the text box to describe what has happened. Please make sure to write enough detail so that the customer can fully understand the situation.
Once you've finished, please click 'Save and close'.
